Year End Forms

Parish Year End Forms were distributed by email to the Corporation of the Parish on January 9th, 2025.  The 2024 forms are due to the Diocesan Office by March 10th 2025. 

The year end forms are the basis of our distribution lists and with the Electoral Synod happening in May 2025 we will need those lists up-to-date so we can ensure the information is flowing effectively from the Diocese and the Search Committee to the Parishes and Synod Delegates.

This year we have 3 different JotForm links. Just like last year the Parish Contact JotForm combines two physical forms (Certificate of Election of Church Wardens & Lay delegates) into one submission, as well as the Parish Statistics JotForm combines two physical forms (General Synod representation and Parish Demographic Statistics) into another submission. The Lay Leadership JotForm is new this year and we are also asking for the email addresses of our Lay Leaders, so as we can communicate learning opportunities and resources to them. You will still need to fill out the Annual Financial Form and email it back to our office.
